Group education seminars run over a six week period, and also give you the opportunity to meet new friends.
We facilitate the safe and early discharge of eligible patients leaving hospital by providing access to care designed to help you gain independence and prevent re-admission to hospital. This program is available in your own home and can provide you with low intensity therapy such as: physiotherapy and podiatry, nursing support and clinical care, personal care and access to a social worker.
Many seniors enjoy staying connected to their church. CatholicCare will work with your parish to link you with their visitation team for a friendly home visit and delivery of Communion at home.
We offer our clients and their families additional emotional and spiritual support through the CatholicCare Chaplain Care Team. Our Chaplains provide personalised, caring and flexible support regardless of your background and beliefs. Our Chaplaincy and Pastoral Care services are optional and confidential. They are also available to any of our clients, their families and carers.
Your ongoing wellbeing is a priority to CatholicCare, we offer at no extra charge a weekly social call or home visit by our kind and caring volunteers.
