Home Care Application Process

Six steps to apply for Home Care Services

Contact us on 13 18 19 or contact us online to arrange a face to face appointment in your home with one of our Home Care Specialists who can guide you through the process.

Step 1 Home Care CatholicCare Sydney
Call our friendly team on 13 18 19 and we can arrange for one of our Home Care Specialists to come and visit you at home to talk you through the process.

Our Home Care Specialists are experts at helping you navigate the My Aged Care system. It doesn’t matter where you are in the process, CatholicCare is here to assist you every step of the way

Step 2 Home Care CatholicCare Sydney
Call 1800 200 422 to let the My Aged Care team know that you would like CatholicCare to deliver your services. Have your Medicare card ready.

For more information on what happens in the call, go to the My Aged Care site. Based on your call, you may need a face-to-face assessment.

Step 3 Home Care CatholicCare Sydney
After your face-to-face assessment, a decision will be made to determine if you're eligible for government funded services and if so, which ones you're eligible for.

This stage is completed by a Home Care Specialist in your home.

Step 4 Home Care CatholicCare Sydney
You may be eligible for interim services under the Commonwealth Home Support Program. We can support you to access these services whilst waiting for a Home Care Package. Call CCareline on 13 18 19 to find out more.

Step 5 Home Care CatholicCare Sydney
When you receive your confirmation letter with your activation number for a Home Care Package from My Aged Care, this is when it is vital that you contact CCareline 13 18 19  as soon as possible.

IMPORTANT NOTE: 30% of seniors leave their confirmation letter sitting in a drawer. After 56 days the activation code expires and you will return to the bottom of the waiting list.

Step 6 Home Care CatholicCare Sydney
A Home Care Specialist will visit your home and help you create a plan that suits your personal needs and budget. Once approved you can start receiving services when you’re ready. Your plan is flexible, so changes can be made any time.
